The Baker Center for Primary Care provides accessible, compassionate and high quality care for your entire family. Our clinic has 6 providers, 3 physicians, 2 nurse practitioners, and 1 certified nurse midwife which work in partnership with you to ensure that your family's medical needs are met. We also include the patients in decision making and take responsibility for the on going care of the patient. From your pregnancy, to caring for your infant, child or adolescent and you as an adult and into your later years. We are truly your medical home for all ages.

Providers

Charles E. Baker, MD Charles E. Baker, MD has been practicing medicine since 1973. Dr. Baker is a graduate of University of North Carolina at Chapel Hill School of Medicine. He completed internship with the University of Vermont at the Medical Center Hospital of Vermont with a specialty in pediatrics. He completed his residency at the University of Pittsburgh School of Public Health Pittsburgh Specialty Hospital with a specialty in pediatrics. He is board certified in American Board of Family Medicine and American Board of Pediatrics.
Amy Chidester, MD Amy Chidester, MD is a graduate of Marshall University School of Medicine in Huntington, WV. She completed her residen- cy at Christ Hospital in Cincinnati, OH with a specialty in family medicine. She completed a fellowship at Florida Hospital in Winter Park, FL with a specialty in OB. She is board certified by the American Board of Family Medicine.
Jennifer Burgart, MD

Jennifer Burgart, MD
Heather Jordan, CNM Heather Jordan, CNM is a 2002 graduate of East Carolina University School of Nursing in Greenville, NC with a Masters of Science in Nursing with a concentration in Nurse Midwifery. Her undergraduate degrees include Bachelor of Science in Nursing from University of Washington and Bachelor of Arts in Cultural Anthropology from Davidson College. She is board certified in American Midwifery Certification Board and American College of Nurse Midwives.
Donna S. Tate, FNP Donna S. Tate, FNP received her Masters of Science in Nursing in 2003 from East Tennessee State University in Johnson City, TN. She received her undergraduate degree in nursing from Winston Salem State University. She has achieved board certification as a Family Nurse Practitioner by the American Nurses Credenttialing Center. Certifications include American Nurses Credentialing Center, American Nurses Association and is board certified in Family Practice.
Ella Markland, FNP Ella Markland, FNP obtained her Masters of Science in Nursing in 2000 from the University of North Carolina at Chapel Hill School of Nursing. She received an undergraduate degree in nursing from John Hopkins University School of Nursing in Baltimore, MD and an undergraduate degree in Political Science from the University of North Carolina at Chapel Hill. Her certifications include American Nurses Credentialing Center and North Carolina Nursing Association.
